GPUT: Remove no-longer-needed permissions and groups

AFS management privileges, now managed in Grouper:
  change_*_quota
  create_*_volume
  create_misc_coll
  create_*_pts_id
  create_user_pts_instance
  ignore_afs_quota_limits
  list_owned_pts_groups
  list_pts_group_members
  list_user_pts_groups
  release_*_volume

afs_admins, misc_coord_role, most_priv_staff
  Only used in AFS privileges above

extra_maint_role, extra_root_role
  These are empty and no longer needed in GPUT

most_priv_princ
  Was used for constructing ACL files from most_priv_staff; no longer needed

see_user_fullname
  This mostly affected the instance manager and possibly some user
  management tools. While the old instnace manager still uses GPUT,
  it is about to be retired and in any case no longer uses this copy.
