rc_net_host: report APEX page in machine-readable CASE ticket info

When the hostinfo web app generates a link to a CASE ticket, part of
the URL is the APEX application page number, which varies depending
on whether this is a new registration request or update (which are
contained in different tables). Report the correct page number in the
machine-readable ticket data, so that the web app can generate the
correct link.
