[COVID - 19] Symptoms Questionnaire

Hello all,

In a research effort to understand the spread of COVID 19, we developed 
a questionnaire to self report symptoms nationwide. We encourage all of 
you to participate. This voluntary questionnaire will take you less than 
5 minutes to complete and will allow to better understand prevalence 
patterns of COVID-19 and similar diseases in your area. Participation in 
this study is voluntary and will not result in any compensation.
